First off, Try to avoid meshsmooth at all costs. Even ban it from your dictionary when modeling for games!
It is much more effective to model the detail in yourself.
You are following the ref quite nicely but there are 2 things you should mind.
First is the joints.

Check out his site www.poopinmymouth.com, he has some very usefull video tutorials!
Second, you should try to fix your polyflow.
Try to make the polyflow "flow" with the shape of muscles.
This is hard to master so the best way is to just take example from experienced/veteran modelers (There are quite some on these forums)
check out their wires and see how they defined certain things.
